Необходимость создания отечественной пневмококковой поливалентной вакцины является одним из основных направлений «Стратегии развития вакцинопрофилактики инфекционных болезней на период до 2035 года».</p></sec><sec><title>Цель</title><p>Цель. Оценка иммунологической эффективности и безопасности Вакцины пневмококковой полисахаридной конъюгированной адсорбированной тринадцативалентной при иммунизации взрослых и детей в Российской Федерации.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Иммунологическая эффективность и безопасность Вакцины пневмококковой полисахаридной конъюгированной адсорбированной тринадцативалентной (ПКВ13) были изучены в проспективном сравнительном рандомизированном двойном слепом исследовании в параллельных группах у взрослых добровольцев в возрасте от 18 до 65 лет и многоцентровом открытом сравнительном исследовании в параллельных группах у детей раннего возраста. В рамках клинического исследования взрослых 60 добровольцев были рандомизированы в две группы по схеме 1:1 (группа I, n = 30; группа II, n = 30). Клиническое исследование ревакцинации у детей включало 119 участников в возрасте 15 месяцев, которые были распределены по схеме 1:1:1 (группа I, n = 39; группа II, n = 40; группа III, n = 40). Референтная вакцина в обоих исследованиях – Превенар®13 (Пфайзер Инк., США).</p></sec><sec><title>Результаты исследования</title><p>Результаты исследования. Клиническое исследование у взрослых показало отсутствие статистически значимой разницы между исследуемыми группами по результатам первичных (доле добровольцев с концентрацией серотипспецифических IgG к каждому из 13 серотипов пневмококка равной или превышающей уровень 0,35 мкг/мл (р &gt; 0,039) до вакцинации и через 4 недели после вакцинации) и вторичных критериев оценки эффективности. Таким образом, доказана сопоставимость иммуногенности ПКВ13 с препаратом сравнения Превенар®13 при однократном внутримышечном введении здоровым добровольцам в возрасте 18–65 лет. Безопасность в обеих группах была также сопоставима, зарегистрировано 15 местных реакций, в том числе 7 НЯ у добровольцев группы I, получивших исследуемый препарат, и 8 НЯ у добровольцев группы сравнения. Статистически значимых различий в частоте зарегистрированных НЯ между группами не выявлено. Аналогичные результаты получены и при иммунизации детей. Для всех 13 серотипов новой ПКВ13 в отношении референтного препарата как первичные, так и вторичные критерии оценки иммуногенности при ревакцинации детей достигнуты (Difference ≤10 %, соотношения СГК и СГТ ≥ 0,5, соответственно), что доказывает не меньшую эффективность Вакцины пневмококковой полисахаридной конъюгированной адсорбированной тринадцативалентной по сравнению с референтной вакциной Превенар®13. В ходе проведения клинического исследования у детей всего было зарегистрировано 27 НЯ, которые имели связь с введением вакцин и были представлены местными и системными реакциями (5 НЯ у участников исследования из группы I (5/39) 12,8%, 18 НЯ у участников исследования из группы II (18/40) 45,0% и 4 НЯ у участников исследования из группы III (4/40) 10%). СНЯ в данном исследовании зарегистрировано не было.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты клинических исследований у детей и взрослых позволяют рекомендовать вакцину ПНЕМОТЕКС® для иммунизации с целью профилактики пневмококковой инфекции, как взрослых, так и детей по схеме 2+1 в рамках Национального календаря профилактических прививок и календаря профилактических прививок по эпидемическим показаниям Российской Федерации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. The need to create a domestic pneumococcal polyvalent vaccine is one of the main directions of the «Strategy for the development of vaccine prevention of infectious diseases for the period up to 2035».</p></sec><sec><title>Aim</title><p>Aim. To evaluate the immunological efficacy and safety of the pneumococcal polysaccharide conjugate adsorbed thirteen-valent (PCV13) vaccine in the immunization of adults and children in theRussian Federation.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The immunological efficacy and safety of the pneumococcal polysaccharide conjugated adsorbed thirteen-valent vaccine were studied in a prospective comparative randomized double-blind study in parallel groups in adult volunteers aged 18 to 65 years and a multicenter open comparative study in parallel groups in young children. In a clinical study of adults, 60 volunteers were randomized into two groups according to the 1:1 scheme (groupI, n = 30; group II, n = 30). A clinical study in children included 119 volunteers aged 15 months who were divided according to the 1:1:1 scheme (group I, n = 39; group II, n = 40; group III, n = 40). The reference vaccine in both studies is Prevenar® 13 (Pfizer Inc., USA).</p></sec><sec><title>Results</title><p>Results. A clinical study in adults showed no statistically significant difference between the study groups according to the results of primary (the proportion of volunteers whose concentration of serotype-specific IgG to each of the 13 pneumococcal serotypes is equal to or exceeds the level of 0.35 mcg/ml (p &gt; 0.039) before vaccination and 4 weeks after vaccination) and secondary criteria for evaluating effectiveness. Thus, the results obtained indicate a comparable level of immunogenicity of PCV13 with the comparison drug Prevenar®13 with a single intramuscular injection to healthy volunteers aged 18–65 years. Safety in both groups was comparable, 15 local reactions were registered, including 7 in group I volunteers who received the test drug, and 8 in comparison group volunteers. There were no statistically significant differences in the frequency of registered adverse events between the groups. Similar results were obtained with the immunization of children. For all 13 serotypes of the new PCV13 in relation to the reference drug, both primary and secondary criteria for assessing immunogenicity during immunization of children have been achieved (Difference ≤ 10% and GMC and GMT Ratio ≥ 0.5, respectively), which proves no less effectiveness of the pneumococcal polysaccharide conjugated adsorbed thirteen-valent vaccine compared with the reference vaccine Prevenar® 13. During the clinical study, a total of 27 adverse events were registered in children, which were associated with the introduction of vaccines and were represented by local and systemic reactions (5 AEs in study participants from group I (5/39) 12.80%, 18 AEs in study participants from group II (18/40) 45.0% and 4 AEs in study participants from group III (4/40) 10%). No SAEs were registered in this study.</p></sec><sec><title>Conclusion</title><p>Conclusion.
