Терапия онкологического заболевания формирует временное иммуносупрессивное состояние, что определяет увеличение частоты и тяжести инфекционных заболеваний. Вакцинация – высокоэффективный и безопасный способ защиты от инфекции, однако у лиц с иммунодефицитом существуют риски ее неэффективности и развития осложнений. Для обоснования необходимости иммунопрофилактики у онкологических больных после проведенной терапии важно понимание сохранности у них специфического ответа после ранее проведенных прививок.</p></sec><sec><title>Цель</title><p>Цель. Оценить сохранность антител к вакциноуправляемым инфекциям у детей с онкологическими заболеваниями после проведенной терапии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Сохранность антител к вакциноуправляемым инфекциям изучена в 3 группах: 1 – у пациентов с онкологическими заболеваниями (n = 62); 2 – у лиц с отсутствием онкологических заболеваний (n = 43), но получивших иммуносупрессивную (ИСТ) и/или полихимиотерапию (ПХТ) и/ или трансплантацию гемопоэтических стволовых клеток (ТГСК), и 3 – у здоровых детей (n = 31 – группа контроля). Концентрацию антител определяли методом иммуноферментного анализа (ИФА). Минимальным защитным уровнем считали количество для кори ³0,18 МЕ/мл, краснухи – ³25 МЕ/мл, гепатита В – ³10 МЕ/мл, дифтерии – 0,03 МЕ/мл и выше. Коэффициент позитивности, оцениваемый как защитный против эпидемического паротита, составлял ³1,0.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Установлено, что лишь от 6,3% до 58,3% детей с онкологическими заболеваниями сохраняют поствакцинальный иммунитет к изученным вакцинным антигенам. Число детей, сохранивших защитный уровень антител в 1-й и 2-й группах было достоверно меньше, чем в группе сравнения. Достоверных различий по уровню защищенных от дифтерии и краснухи не выявлено. Максимальное влияние на утрату антител оказывает проведенная ТГСК. Для дифтерийных и краснушных антител различия не выражены. Была проанализирована возможная связь генетических поломок у 35 обследованных детей с онкологическими заболеваниями и сохранностью антител. Оказалось, что при наличии хромосомных делеций в 100% случаев утрачены антитела к кори и в 75% – к дифтерии, что отличалось от других хромосомных аномалий.</p></sec><sec><title>Заключение</title><p>Заключение. На сохранность антител у пациентов с онкологическими заболеваниями в анамнезе оказывает влияние наличие в терапии ТГСК, тип генетической поломки, а также особенность антигена вакцины. Дети с онкологическими заболеваниями, а также с неонкологическими, но получившие терапию ИСТ, ПХТ, ТГСК, должны быть вновь привиты против вакциноуправляемых инфекций, несмотря на указание о наличии прививок до терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Cancer therapy forms a temporary immunosuppressive state, which determines an increase in the frequency and severity of infectious diseases. Vaccination is a highly effective and safe way to protect against infection, but people with immunodeficiency have risks of inefficiency and complications. To substantiate the need for immunoprophylaxis in cancer patients after therapy, it is important to understand the preservation of their specific response after previous vaccinations.</p><p>The aim of the study was to assess the safety of antibodies to vaccine–controlled infections in children with oncological diseases after therapy</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The safety of antibodies to vaccine-controlled infections was studied in 3 groups: 1 -in patients with oncological (n=62); 2-in the group (n=43) withoutoncological diseases, but who received immunosuppressive (IST) and/or polychemotherapy (PCT) and/or hematopoietic stem cell transplantation (HSCT), and 3– in healthy children (n=31 – comparison group). The concentration of antibodies was determined by the ELISA method. The minimum protective level was considered to be the amount for measles ³0.18 IU/ml, rubella - ³25 IU/ml; hepatitis B - ³10 IU/ml; diphtheria – 0.03 IU/ml and higher. The coefficient of positivity, estimated as protective against mumps, was ³1.0.</p></sec><sec><title>Results</title><p>Results. It was found that from 41.7% to 93.7% of children with cancer lose post-vaccination immunity to the studied vaccine antigens. The number of children who retained the protective level of antibodies in groups 1 and 2 was significantly less than in the comparison group. There were no significant differences in the level of those protected from diphtheria and rubella. The maximum effect on the loss of antibodies is provided by the performed HSCT. For diphtheria and rubella antibodies, the differences are not pronounced. The possible connection of genetic breakdowns in 35 examined children with oncological diseases and the safety of antibodies was analyzed. It turned out that in the presence of chromosomal deletions, antibodies to measles were lost in 100% of cases and to diphtheria in 75%, which was different from other chromosomal abnormalities.</p></sec><sec><title>Conclusion</title><p>Conclusion. The safety of antibodies in patients with a history of cancer is influenced by the presence of HSCT in therapy, the type of genetic breakdown, as well as the peculiarity of the vaccine antigen.
