Корь остается одной из самых контагиозных инфекций в мире; несмотря на наличие эффективной вакцины, вспышки заболевания продолжают регистрироваться как в странах с низким, так и с высоким охватом иммунизацией. Пандемия COVID-19 вызвала сбои в работе систем здравоохранения и привела к накоплению восприимчивого населения, что делает актуальным сравнительный анализ эпидемического процесса в странах с различными социальными и климатическими условиями, такими как Вьетнам и Российская Федерация, для понимания общих закономерностей и региональных особенностей.</p><p>Цель исследования – сравнить основные эпидемиологические характеристики кори во Вьетнаме и Российской Федерации в период с 2000 по 2024 гг.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Было проведено описательное эпидемиологическое исследование на основании статистических данных из открытых источников ВОЗ (JRF, WUENIC) и национальных систем эпидемиологического надзора. Проанализированы показатели заболеваемости на 100 тыс. населения, показатели охвата вакцинацией одной и двумя дозами вакцины корь-паротит-краснуха (КПК).</p></sec><sec><title>Результаты</title><p>Результаты. В обеих странах сохраняется цикличность эпидемического процесса с интервалами 2–5 лет. Во Вьетнаме на фоне высокого (94 ± 3,33 %), но нестабильного охвата вакцинацией отмечается общая тенденция к снижению заболеваемости после 2007 г. В РФ при более стабильном и высоком охвате (97,9 ± 0,26 % для КПК-1), начиная с 2008 года, наблюдается фаза роста заболеваемости (среднегодовой темп прироста 25,2 %), достигшая максимума за весь период наблюдения в 2024 году (15,51 на 100 тыс.). Возрастная структура заболеваемости в обеих странах характеризуется преимущественным поражением детей до 1 года и 2–4 лет, однако в РФ в последний подъем вовлекались также дети 5–9 лет. Выявлена относительная синхронность подъемов и спадов заболеваемости в двух странах, включая спад в период пандемии COVID-19 (2020–2022) и последующий резкий рост в 2023–2024 гг.</p></sec><sec><title>Заключение</title><p>Заключение. Несмотря на различия в схемах вакцинации и климате, эпидпроцесс кори во Вьетнаме и РФ характеризуется сходной цикличностью. Стабильно высокий средненациональный охват вакцинацией не гарантирует отсутствия вспышек и может маскировать локальные пробелы в иммунитете. Рост заболеваемости в РФ при высоком охвате может быть связан с накоплением неиммунных прослоек среди взрослых, завозными случаями и дефектами вакцинации в отдельных группах. Полученные данные обосновывают необходимость мониторинга иммунизации на субнациональном уровне, «наверстывающей» вакцинации и рассмотрения корректировки календаря прививок (в сторону более раннего начала вакцинации) для снижения заболеваемости среди детей младшего возраста</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Measles remains one of the most contagious infections in the world; despite the availability of an effective vaccine, disease outbreaks continue to be reported in both lowand high-immunization-coverage countries. The COVID-19 pandemic caused disruptions in healthcare systems and led to the accumulation of a susceptible population, making a comparative analysis of the epidemic process in countries with different social and climatic conditions, such as Vietnam and the Russian Federation, relevant for understanding common patterns and regional features.</p></sec><sec><title>Aim</title><p>Aim. To compare the main epidemiological characteristics of measles in Vietnam and the Russian Federation from 2000 to 2024.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. A descriptive epidemiological study was conducted based on statistical data from open sources of the WHO (JRF, WUENIC) and national epidemiological surveillance systems. Incidence rates per 100,000 population, coverage rates for one and two doses of the measles-mumps-rubella (MMR) vaccine were analyzed.</p></sec><sec><title>Results</title><p>Results. In both countries, cyclical patterns in the epidemic process persist with intervals of 2–5 years. In Vietnam, against a background of high (94 ± 3.33 %) but unstable vaccination coverage, a general trend towards decreasing incidence after 2007 is observed. In the Russian Federation, with more stable and high coverage (97.9 ± 0.26 % for MMR-1), an increasing incidence phase has been observed since 2008 (average annual growth rate of 25.2 %), reaching its maximum for the entire observation period in 2024 (15.51 per 100,000). The age structure of incidence in both countries is characterized by predominant involvement of children under 1 year and 2–4 years; however, in the Russian Federation, the most recent upsurge also involved children aged 5–9 years. A relative synchronicity of rises and declines in incidence was identified between the two countries, including a decline during the COVID-19 pandemic (2020–2022) and a subsequent sharp increase in 2023–2024.</p></sec><sec><title>Conclusion</title><p>Conclusion. Despite differences in vaccination schedules and climate, the measles epidemic process in Vietnam and the Russian Federation is characterized by similar cyclical patterns. Consistently high national average vaccination coverage does not guarantee the absence of outbreaks and may mask local immunity gaps. The increasing incidence in the Russian Federation, despite high coverage, may be associated with the accumulation of non-immune populations among adults, imported cases, and vaccination deficiencies in certain groups.
