Вторичное распространение коклюша в школах обусловлено угасанием иммунитета к возрасту 6–7 лет, низкой настороженностью у медицинских работников относительно коклюшной инфекции у детей старше 5 лет и тесными контактами в организованных коллективах. Объем и качество противоэпидемических мероприятий играют ключевую роль в быстрой локализации очагов инфекции.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность противоэпидемических мероприятий в групповых очагах коклюша в школах отдельных субъектов Российской Федерации.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы данные 36 территориально и временно не связанных групповых очагов коклюша в 9 субъектах РФ в 2023 г. (период максимального подъема заболеваемости за 26 лет). Использованы отчетные формы и разработанные оценочные таблицы, включавшие параметры длительности очага, числа заболевших и контактных, охвата лабораторным обследованием, а также сроков и объемов применяемых мер (включая перевод на дистанционное обучение).</p></sec><sec><title>Результаты</title><p>Результаты. Средняя длительность регистрации очагов составила 41 день (от 21 до 88 дней). Во всех очагах своевременно разворачивались противоэпидемические мероприятия (изоляция, медицинское наблюдение, проветривание). Перевод на дистанционное обучение применялся в 14 очагах. Выявлена прямая корреляция между числом заболевших и количеством классов, переведенных на дистанционное обучение (rs = 0,63, p &lt; 0,001), а также сроками этого перевода (rs = 0,53, p = 0,001). Это указывает на то, что к переводу на дистанционное обучение прибегали вынужденно – в крупных и длительно существующих очагах, когда стандартные меры ограничения уже не давали результатов.</p></sec><sec><title>Заключение</title><p>Заключение. Длительное существование и масштаб очагов свидетельствуют об отсутствии иммунной прослойки среди школьников. Перевод на дистанционное обучение действенен как противоэпидемическое мероприятие только на ранних сроках формирования очага. Необходимо применение в практике ключевых критериев оценки эффективности противоэпидемических мероприятий в очагах коклюша. Медицинское наблюдение и лабораторное обследование контактировавших лиц критически важны для раннего выявления больных и их изоляции из коллектива.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Secondary spread of pertussis in schools is driven by waning immunity by the age of 6–7 years, low clinical vigilance among healthcare workers regarding pertussis in over 5 years old, and close contact within organized groups. The scope and quality of anti-epidemic measures play a key role in the rapid containment of infection outbreaks.</p></sec><sec><title>Aims</title><p>Aims. To evaluate the effectiveness of anti-epidemic measures in cluster outbreaks of pertussis in schools across several regions of the Russian Federation.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. Data from 36 geographically and temporally unlinked cluster outbreaks of pertussis across 9 regions of the Russian Federation in 2023 (the period of the highest incidence peak in 26 years) were analyzed. Standard reporting forms and custom-developed evaluation tables were utilized, capturing parameters such as outbreak duration, number of cases and contacts, laboratory testing coverage, and the timing and scope of implemented interventions (including the transition to remote learning).</p></sec><sec><title>Results</title><p>Results. The mean duration of outbreak monitoring was 41 days (range: 21 to 88 days). In all outbreaks, anti-epidemic measures (isolation, medical surveillance, and room ventilation) were implemented promptly. The transition to remote learning was implemented in 14 outbreaks. A positive correlation was found between the number of cases and both the number of classes switched to remote learning (rs = 0.63, p &lt; 0.001) and the timing of this transition (rs = 0.53, p = 0.001). This indicates that remote learning was resorted to out of necessity in large, prolonged outbreaks, when standard containment measures had already failed to yield results.</p></sec><sec><title>Conclusions</title><p>Conclusions. The prolonged duration and scale of outbreaks indicate a lack of immune protection among schoolchildren. Transitioning to remote learning is effective as an anti-epidemic measure only during the early stages of an outbreak. Monitoring and validation of clear criteria for evaluating the effectiveness of anti-epidemic measures in pertussis outbreaks are required.
