Генерализованная форма менингококковой инфекции (ГФМИ) является одним из наиболее опасных инфекционных заболеваний. ГФМИ характеризуется стремительным течением и высоким риском летальных исходов. Наибольшему риску подвергаются дети первого года жизни, начиная с первых ее месяцев. Оценка безопасности и эффективности совместного применения менингококковой конъюгированной вакцины MenACWY-TT с другими педиатрическими вакцинами необходима для возможности применения в программах иммунизации детей первого года жизни. Цель. Оценка профиля иммуногенности и безопасности вакцины MenACWY-TT, применяемой по схеме 2+1 (вакцинация в 3 и 6 месяцев, ревакцинация в 12 месяцев), при одновременном введении с вакцинами Национального календаря профилактических прививок у здоровых детей в Российской Федерации. Материалы и методы. Проведено открытое рандомизированное многоцентровое исследование иммуногенности и безопасности четырехвалентной менингококковой вакцины, конъюгированной со столбнячным анатоксином, MenACWY-TT, с активным контролем (III фаза). Исследование осуществлялось в 8 центрах Российской Федерации. Иммуногенность и безопасность изучались при введении вакцины по схеме 2+1 детям в возрасте 3, 6 и 12 месяцев одновременно с вакцинами Национального календаря профилактических прививок. В исследование были включены 225 здоровых младенцев, которые были распределены в соотношении 2:1 в группу MenACWY-TT + плановая вакцинация (n = 150) и группу сравнения – только плановые вакцины (n = 75). Иммуногенность против менингококков серогрупп A, C, W и Y определялась методом бактерицидной активности сыворотки с человеческим комплементом (чСБА), а у части вакцинированных – дополнительно с кроличьим комплементом (кСБА). Безопасность оценивалась на протяжении 30 дней после каждой инъекции; наблюдение за развитием серьезных нежелательных явлений продолжалось до финального визита – 30-й день после ревакцинации в возрасте 12 месяцев. Результаты и обсуждение. Через 30 дней после ревакцинации в возрасте 12 месяцев доля участников с серопротекцией (чСБА ≥1:8) в группе MenACWY-TT составила 89,6 % (серогруппа A), 82,3 % (серогруппа C) и 80,2 % (серогруппы Y и W); в post-hoc анализе без учета одного центра (в котором регистрировались значительно отличающиеся от других центров показатели) – ≥93,8 % для всех серогрупп. После первичной серии вакцинации (в 6 мес.) серологический ответ достигнут у 70,1–94,8 % участников. Иммуногенность плановых вакцин при совместном введении была сопоставима с группой сравнения. Местные реакции на MenACWY-TT (эритема 7,4 %, болезненность 6,0 %) были легкой или средней степени выраженности. Серьезные нежелательные явления зарегистрированы у 2,7 % участников в обеих группах, связь с вакцинацией не установлена. Заключение. Вакцина MenACWY-TT при введении по схеме 2+1 с первых месяцев жизни продемонстрировала хорошую иммуногенность и благоприятный профиль безопасности при одновременном введении с педиатрическими вакцинами Национального календаря профилактических прививок для плановой иммунизации детей первого года жизни.</p></abstract><trans-abstract xml:lang="en"><p>Relevance. Invasive meningococcal disease (IMD) is one of the most dangerous infectious diseases due to its rapid progression and high risk of fatal outcomes, with infants in their first year of life, starting from the first months, being at greatest risk. Evaluation of the safety and efficacy of co-administration of meningococcal conjugate vaccine MenACWY-TT with pediatric vaccines is necessary for its potential use in the immunization program for infants. Aims. To evaluate the immunogenicity and safety profile of MenACWY-TT vaccine administered according to a 2+1 schedule (vaccination at 3 and 6 months, booster at 12 months), when co-administered with National Immunization Schedule vaccines in healthy children in the Russian Federation. Materials and Methods. An open-label, randomized, multicenter, active-controlled Phase III study was conducted at 8 centers in the Russian Federation. The immunogenicity and safety of MenACWY-TT immunization according to a 2+1 schedule at 3, 6, and 12 months of age co-administered with National Immunization Schedule vaccines, were studied. The study enrolled 225 healthy infants who were randomized in a 2:1 ratio to the MenACWY-TT + routine vaccines group (n = 150) or the comparator group receiving routine vaccines only (n = 75). Immunogenicity against meningococcal serogroups A, C, W, and Y was determined by serum bactericidal assay using human complement (hSBA), and in a subset of vaccinated participants, additionally with rabbit complement (rSBA). Safety was assessed for 30 days after each injection; monitoring for serious adverse events (SAEs) continued until the final visit—30 days after the booster dose at 12 months. Results. At 30 days after the booster dose (12 months), the proportion of participants with seroprotection (hSBA ≥ 1:8) in the MenACWY-TT group was 89.6 % (serogroup A), 82.3 % (C), and 80.2 % (Y and W); in post-hoc analysis excluding one center (at which significantly different values were recorded compared to other sites) – ≥ 93.8 % for all serogroups. After the primary series (at 6 months), serological response was achieved in 70.1–94.8 % of participants. The immunogenicity of routine vaccines when co-administered was comparable to the comparator group. Local reactions to MenACWY-TT (erythema 7.4 %, tenderness 6.0 %) were mild or moderate in severity. Serious adverse events (SAEs) were reported in 2.7 % of participants in both groups, with no established relationship to vaccination. Conclusion.
