Зарубежными и отечественными авторами в течение последних тридцати лет отмечается значительное снижение заболеваемости менингококковой инфекцией в отличие от бактериальных менингитов не менингококковой этиологии. При этом S. pneumoniae занимает одно из ведущих мест в этиологической структуре бактериальных менингитов по РФ Цель – обоснование комплексного подхода к диагностике пневмококкового менингита у взрослых с учетом клинико-эпидемиологических особенностей заболевания и высокочувствительных лабораторных тестов Материалы и методы. Проведен ретроспективный анализ 38 историй болезни пациентов, находившихся на стационарном лечении в Красноярской межрайонной клинической больнице скорой медицинской помощи имени Н. С.Карповича по поводу бактериального менингита в течение 2015 по 2017 гг. Результаты. Среди госпитализированных с диагнозом «бактериальный менингит». Лидирующая роль в этиологии заболевания принадлежала Streptococcus pneumoniae (55,2%). В остальных случаях наблюдалось поражение оболочек мозга Staphylococcus aureus, Streptococcus spp, грибами рода Candida spp., E. сoli, Neisseria meningitidis, Cryptococcus spp., некоторые бактерии обнаруживались в ассоциациях с другими видами бактерий либо вирусами, в 7,8% возбудителя выявить не удалось Заключение. Микробиологическая диагностика бактериального менингита требует применения комплекса методов, включающего не только микроскопическое, бактериологическое и серологическое исследование (латекс-агглютинацию), но и ПЦРдетекцию патогенных микроорганизмов в спинномозговой жидкости, диагностическая ценность которой составляет 46,4%.</p></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Foreign and domestic authors notice that over the past thirty years there has been a significant reduction in the incidence of meningococcal infection, in contrast to bacterial meningitis not meningococcal etiology. S. pneumoniae occupies one of the leading places in the etiological structure of bacterial meningitis in the Russian Federation The purpose is of substantiation of the complex approach to the diagnosis of pneumococcal meningitis in adults, taking into account the clinical and epidemiological features of the disease and highly sensitive laboratory tests Materials and methods. Was conducted a retrospective analysis of 38 case histories of patients who were hospitalized at the Krasnoyarsk Interdistrict Clinical Emergency Hospital named after NS Karpovich with bacterial meningitis during 2015 to 2017. Results. Among those hospitalized with the diagnosis of «bacterial meningitis» the leading role in the etiology of the disease belonged to Streptococcus pneumoniae (55.2%), in other cases: Staphylococcus aureus, Streptococcus spp, fungi of the genus Candida spp., E. coli, Neisseria meningitidis, Cryptococcus spp., Some bacteria were detected in associations with other bacterial species or viruses, in 7.8% cases the pathogen was not detected. Conclusion.
