В обзоре освещаются современные представления о дифтерийном бактерионосительстве как о бессимптомном инфекционном процессе без участия в нем дифтерийного токсина. Инфекционный процесс осуществляется за счет бактериальных факторов колонизации, имеющихся как у токсигенных, так и у нетоксигенных дифтерийных штаммов. Носительство токсигенных штаммов возникает при заражении лиц, обладающих изначально защитным уровнем антитоксических антител и возрастающим далее в процессе носительства. Антитоксический иммунитет не является препятствием для возникновения носительства. При заражении нетоксигенным штамммом исходный уровень антитоксина не имеет значения – нетоксигенные штаммы неспособны вызвать заболевание дифтерией. Носительство возникает при заражении лиц, не обладающих местной и общей иммунологической защитой к бактериальным факторам колонизации (так наз. антибактериальным иммунитетом, в противовес антитоксическому); выработка антибактериальной защиты приводит к освобождению от носительства. Пока что уменьшать распространение носительства удается только косвенным путем – благодаря созданию искусственного антитоксического иммунитета, резко снижающего число больных дифтерией – основных источников возбудителя. В настоящее время известна природа многих поверхностных бактериальных структур, ответственных за колонизацию (адгезию и инвазию бактерий). Изучение иммуногенных свойств этих структур может лечь в основу иммунопрепаратов, создающих иммунологическую защиту против колонизации, то есть против носительства.</p></abstract><trans-abstract xml:lang="en"><p>The diphtheria carriage is a asympthomatic colonization of oro- and nasopharynx by toxigenic and nontoxigenic Corynebacterium diphtheriae. The carriage of toxigenic strains prevents a complete eradication of diphtheria infection in spite of mass toxoid immunization. The contamination by toxigenic diphtheria bacteria leads to the carriage if the person has a protective level of diphtheria antitoxin. Contamination with the toxigenic and nontoxigenic leads to the carriage if the person has no protection to the bacterial colonization factors.
